 Street---- Father's Occupation---- Mother's Maiden Name---Color--- Age---
 Mother's Residence---- Mother's Birthplace--- Mother's Occupation-- Name 
and
 address
 of Physician (or other person) reporting said birth----Date when received 
by
 Town Clerk----
