Hi Rick,
 
Using your lathe*, cut a groove in the crank rod.  Then,  fit the  tube 
handle in place, heat it, and roll it with a dulled pipe cutter  which will provide 
the crease that holds it.  You'll need a torch (propane  is adequate), vise 
to hold the handle, and vise grips will do nicely for  turning the tube in the 
pipe cutter.  If you wish to be really uptown,  knurl the tube on your lathe 
before fitting it to the crank.  Finally, for  the excessively anally 
compulsive, drill, bore, and thread a recessed zerk  fitting in the handle end to use 
for applying just that dab** of grease that  keeps the handle tube turning 
freely.

with first a triangle and then a round file, cut it to a sufficient depth.
**a unit of measure that is slightly smaller than the normal,  larger amount 
of grease that comes out and gets all over everything.
